Please use this checklist when applying for IMRF retirement benefits.
IMRF. File this form one month before your retirement date.
This application is for a total refund of your IMRF membercontributions. You should file this form ONLY if you are nolonger working, in any position, for the employer withwhom you made these contributions AND if you are nolonger working for any employer in an IMRF-qualifyingposition.
It is important to periodically verify your beneficiaries. If you are unsure, complete a form with your most updated information.
You may change your name, address, telephone number and marital status through Member Access or using this form.
If you have received an IMRF separation refund and have
forfeited your IMRF pension credits (service), you may
have those credits restored. Please read the form for situations that qualify.
Use this form when the member was not enrolled, and earnings and contributions for the member were not reported to IMRF.
